Many of our clients prefer to order appraisals from us via fax. Faxing has until now had its drawbacks: the transmission may not come through clearly or pages get shuffled and lost. You might misplace our fax number, not have an order form handy, or not want to take the time to make out a cover sheet.
With my faxable order form there’s a barcode that prints out on the order form. When you fax it, a special software application reads the barcode. A digital copy of the order form is routed right to a special directory in our system, specific to that order, and we’re notified it’s arrived (even if we’re out of the office!).
Better yet: The software reads the subject information and gets it ready for our form filling application, so we get a running start. If we do need to call for more information, we know immediately, without your fax sitting on the fax machine waiting for someone to pick it up and type it in.
Your LOS might have ready-to-print appraisal requests already. No problem – use that instead! Just download a cover sheet from here. It has a similar barcode that will accomplish everything our own order form does. Print it out, make it the first page of your fax, and we’re in business!
Don’t want to visit our website every time you need to order an appraisal? Just print out order forms or cover sheets in advance and keep them by the fax machine. Filling out our order form by hand works, too. A digital copy still gets delivered to our system and we can start the report promptly.

Information we receive from a consumer reporting agency such as a credit history.
4. What information we may disclose:
OPTION #1: We do not disclose nonpublic personal information about clients or former clients except as permitted by law. (If you select this option and you in fact don’t share information with anyone, then your form is done – if you share information as permitted under one of the exceptions, i.e., as permitted by law, then you will need to include that disclosure. See, for example part C(3) of the article on preparing a disclosure form.)
OPTION #2: We may disclose the nonpublic personal information about you described above, primarily to provide you with the appraisal services you seek from us.
5. Who we share information with: Unless you tell us not to, we may disclose nonpublic personal information of the type described above about you to the following types of third parties:
·Financial service providers as they pertain to your business, such as banks and lending institutions, estateplanners, mortgage brokers, tax attorneys, etc.
·Non-financial companies as they pertain to your business, such as retailers, direct marketers, etc.
·We may also disclose nonpublic personal information about you to nonaffiliated third parties as permitted by law. (If you include this provision then you will also need to include another section identifying the types of nonaffiliated third parties to whom you are referring in this provision, See, for example part C(3) of the article on preparing a disclosure form.)
Opt-Out-Notice
·You may limit information shared about you. If you prefer that we not disclose nonpublic personal information about you to third parties, you may opt out of those disclosures, that is, you may direct us not to make those disclosures (except those permitted by law).
